Pitcher/DH

Last night I was scoring a college game and one player was listed as the pitcher/DH He pitched the first 5 innings then was replaced on the mound but stayed in as the DH. How would one score this or set it up when entering the starting lineup?

You would create him as the Pitcher so he gets credit for pitching the game. A player can not be in two positions, so the fact that he is DH is irrelevant... he is a Pitcher that is also hitting in the game. When the new pitcher comes into the game, just do the Defensive Substitution only to sub in the new pitcher, but leave the original pitcher in the Offensive lineup.
